Does it really sit off the shoulder?

Yes. The neckline is cut deliberately wide, so it naturally drops lower on one side as shown in the photos. If you want a slightly easier, more slouched look, size up.

Is the white fabric very sheer?

The fabric is light rather than heavy. In the close-up image, a dark strap shows through at the shoulder, so nude or seamless underwear will give the cleanest finish.

Can I wear a bra with the open back?

You can, but the wide neckline and low back mean standard straps may stay visible. A bandeau, bralette or low-back bra works best with this shape.
